"Mandatory Medical Forms Checklist" Question

    I have a checklist of medical forms I should bring to inprocessing, which I received in an email from USAFA Admissions. It's a very helpful tool, but I'm not recognizing three of the items on it; the "Airmanship Program 1042" form, the "in-processing TSWF Encounter Worksheet", and the "Pharmacy Cadet In-Processing Form"

    I've looked in my portal for them, but I haven't seen any forms I've filled out that might correlate to these items on my list.

    Are the forms with "In-processing" in the name to be gained on I day? And what sort of information does the "Airmanship Program 1042" form contain?

    The three forms you mention were supposed to have been downloaded from your portal and printed before you clicked submit. Once you click Submit you can no longer download the forms. If you forgot to print them out before you clicked submit, I recommend you contact your USAFA counselor.

    Oooh, I see! I thankfully seem to have 2 of them, they're names just weren't obvious from the portal's naming system. :rolleyes:

    "Pharmacy Cadet In-Processing Form" + "Airmanship Program 1042" = "Airmanship Programs/Medical Screening" in the portal :smile:


    However, I'm still confused as to what the "TSWF Encouter Worksheet" could be...is it that 127 page security clearance questionnaire? :confused:

    No, it's also from the Airmanship/Medical Screening form on the Appointee Checklist. Don't worry if you've misplaced it, however, as you will be able to print the forms again from the "To Bring" checklist screen.

    The TSWF is the very last page of the Airmanship packet of pages in this part of the portal. Its where you will notate anything you have in the packet. The letters are at the top of the page, look to the back of the Airmanship form...
