Meaning of the waiver

If the academy approves your medical waiver, is that a highly positive sign that you are very competitive or just another step in the process?
 
If you mean that WP has requested a waiver, then yes, you are a competitive candidate.

If your medical waiver was granted, that is done by the docs at WP and they have no idea of your competitiveness. They only look at your DQ and medical file and make the determination as to whether they recommend the waiver (granted) or not recommend the waiver (not granted) to the admissions committee.
Of course, the fact that the RC asked for a waiver for you indicates that you are competitive, but the fact that you were granted the wavier does not mean that you are more competitive.
