Medical History - all medications for life?

I am a mom helping my son fill out his medical history. We just got the DODMERB letter yesterday. From reading this forum, it sounds like we are to disclose every medicine ever taken, including an antibiotic for ear infections when a toddler. Is this correct? Or do we just disclose any current medications or medications in the recent past/high school era? Any clarifications would be helpful. Thanks
 
I'm not too sure about the need to disclose antibiotic use at age 3, but you will be asked questions on the form that begin with "Have you ever...."

"Ever" is along time, and would include birth to present.

For example, Question #41 is: "Have you ever had or do you now have: Hepatitis?"

Getting hepatitis from a neo-natal ICU blood transfusion would probably need to be reported.

But in answering #53 - ("Have you ever had or do you now have Back Pain or Trouble?") you could probably answer "No" if you happen to remember a sore back after chopping and stacking 6 cords of wood one weekend.

The question specifically says: "Have you ever had or do you now have....take any medications regularly." So, it means what it says from birth topresent, but it also means maintenance medications....insulin, Adderall, synthroid, etc. Antibiotics are used to treat infections and are short term therapy. :thumb:

If you took Accutane for 5 months to treat acne but discontinued use 10 months ago and do not anticipate taking it again, is that considered "regularly" and does it need to be disclosed?
