Medical Records

For the medical portion of the SA application process, what are required in reference to medical records. Do we need to get copies of records from all doctors, eye doctors, dentists, etc...? How far back should we go? We moved when my daughter was 7. She has never had anything other than normal childhood things. Do I need to track down those pediatrician records? Any advice is appreciated. Thanks.

The only time prior medical records would be required is if DoDMERB has requested them. DoDMERB will first review the Report of Medical History to determine if any records are required. They are not needed for the medical examination.
