The application did not come with Son's permit to report packet. I called their 800 number and they didn't know what I was talking about. I asked to be connected to the USNA branch in Annapolis, MD (yes, I had to tell the operator in what city and state the USNA was located). The woman that I talked to at the branch knew exactly what I was talking about and is sending the Midshipmen Membership Application in the mail today.

If you didn't get an application, call the 800 number and ask to be connected to the USNA branch. The service at the branch was excellent.

BTW - I was also told that they sent a stack of forms to USNA for inclusion in the permit to report packets and have since found out that "a number" of them were not included in the packets.
 
Thanks for the tip, MIHOSER! Our son didn't get the NFCU form either. I called 1-888-842-6328 and had to spend several minutes convincing them to let me talk to someone at the USNA branch (they said the branches don't "take calls" :rolleyes:). After being transferred from the operator to the membership dept., I explained again what I needed (and, yes, had to tell them USNA was in Annapolis MD, :rolleyes: again!). The lady said she did not have access to the necessary form and put me on hold to call the USNA branch herself, then came back to tell me that someone at the branch would indeed talk to me - yea! Rebecca at the USNA branch took my call at that point, and she knew exactly what I needed, promised to mail it today, and was all around helpful and lovely. So, once you get through the gauntlet, it's all good. Those of you who received this particular form in your PRT - be grateful! :thumb:
 
Whew! I AM grateful that we got the form and didn't have to do the extra work. We are still trying to get one of the police record checks finished. We sent in all the info, payment, plus a clarification letter initially. They returned everything with a list of requirements for different services. One item was highlighted on the list. We fulfilled that requirement, and returned everything, only to get it back a week later with another item highlighted on the same list. This has happened three times; each time a different requirement is highlighted. Couldn't they just have highlighted ALL the ones we needed to do the first time? This time I wanted to send an addendum to the original letter that says, "We just need you to check two boxes on the enclosed form, stick it in the enclosed addressed and stamped envelope and mail it" but I thought better of it:rolleyes: Mailed it out again today...Hopefully we got everything right this time!
