I go to a school outside my congressional district so do I ask for a nomination from the Congressman that I am under or the Congressman my school is under? And this might sound stupid but how should I refer to him by (Sir, the Honorable, etc)

You need to apply to the congressional district where your legal residence is. You address a congressman/congresswoman as "Dear Congressman (or woman) X: For your senator, you would say: "Dear Senator X."
