Orthodontic Questionnaire

I was asked by the Naval Academy to fill out an Orthodontic Questionnaire so i printed out the form, asked my orthodontist to fill it out, and then sent it. A few days ago I received the same form on the mail. How long should my form take to get processed, or should I fill it out again and mail it again?

I would call or email whatever contact information there is on the form and ask them if they have received your prior form. My guess is the two letters crossed in the mail system.
