Permit to Report Packet

Police Record Check

Son emailed his RD and asked if he should submit a second record check now that he has turned 18. This is the response he received:

"Since we have one on file already you don't need to submit another form. If for some reason things change, you will need to email us and let us know of the changes."

So..we are not going to bother with getting another form and taking it to the police station for submission to the academy.
 
I put Active Duty for the Military Status and i left the pay grade blank. And our rank would be "MIDN" right?
 
My son's application form was specifically titled "Midshipman Membership Application" and already had the rank, pay grade, branch of service, duty activity, etc., pre-printed on the form. You might want to call the toll-free NFCU phone number and ask if they can mail you Form NFCU 97MD (1-10). A lot of the information they ask for on the regular membership form is not required on the Midshipman Membership Application. Hope this helps.
 
Mailed in my manila envelope on Saturday! Also finished all my medical (too many shots) and sent that. It is good to have the paperwork finished for now. How is everyone else doing with the PTR packet?

We have been struggling with the immunizations - son's doctor couldn't get the IPV adult booster so we had to go to a Travel MD that specializes in immunizations.

Had three shots today, but the doctor said we should hold off with the TDap since son has three state tournament games this week and this usually causes a sore arm :)

Hoping to have it all done soon.

It's coming so quickly now, I'm happy and sad at the same time :)
 
Daughter got the immunizations done (just had to have HepA), got the note from the doctor about her meds, BUT has to have wisdom teeth out on June 1.

So much to do, so little time.
 
Remember...you don't have to remove the wisdom tooth if it is not a problem at this point...have the dentist have the final say! :cool:

I've barely been able to glance at it...I tested positive on the TB test so I've been running around getting x-rays and begging doctors to get my results stat...

Only 44 more days left to I-Day
 
Wisdom Teeth

JBow: My DD had hers out last Wednesday, she had no problems after treatment except for the swelling! She's looking more normal today. Hopefully your DD will have similarly easy recovery. The only downside was not being able to run or go to her work-out with the trainer because she said it felt like she was carrying waterbags on her face!

We called Dr. Worm in the Dental Office at USNA, he advised that if she waited and the wisdom teeth became a problem while there, she would be well taken care of, but if she had a heavy academic or athletic schedule, the interruption might be very difficult to deal with. So, we decided not to complicate her life more while there and have the teeth extracted at home where I could help her out with ice packs and cool soup.

Hope all works out for her!
 
i know that our plebes will get their room assignments on Induction day.....will that be the same time their moms (missing him already) will get the info so that we (ok me) can start sending them letters from home ?
 
Trying to track all of son's travel in last 7 years......argh. He's been to about 7 countries, several times over.....and his old passport had been through the wash and I can't read the visas!!!

...praying that TB test comes back negative since he had a BCG innoculation (for TB) when he was 2. Staring at his arm, willing it not to swell....

Other than that.....1-1/2 weeks until graduation, prom last week, orchestra concerts, NJROTC banquet, congressional youth conference....how many more activities can we cram in the last week of school????????? Getting son to be home, much less sit down long enough to fill out a form has been challenging.
 
Final Grades

I was looking through my PTR packet and didn't find anything about sending in our final grades. I was assuming something like that would be in there. Dose anyone know what we are supposed to do for that?
 
Bailey: You should get a letter.....seems like it came mid June last year....telling what company your plebe will be in and how to address mail. The initial address is for the summer only. Then they get a box and are simply MIDN XXX, Box XXX, Annapolis, MD 21412. Their first address will include company and platoon.
 
explode2014, I am just sending the grades as I did for last semester. I do have one address with two different "to the attention of" lines, one for Candidate Guidance Office (which is where we sent transcripts in the past) and one for Dean of Admissions (which is where we sent the police record checks). Anyone know which of these we should use now, or if it matters??
 
So I walked into my room yesterday and saw all of my papers sitting on top of my desk, so I decided to look through them again.

It was then that I realized that most of the paperwork is due today and I pretty much freaked out. All of the paperwork was pretty much done - the only thing I have to worry about is getting my immunization papers filled out.

Quite a big shock to me. I feel incredibly dumb for not checking earlier to see when they were due. I don't know why, but for some reason I just assumed that I had a lot more time than I did.
 
nsiderbam, after all the interminable months of waiting, now the days are flying by. I hope you get everything turned in without any trouble.

Singaporemom, did that TB test come back negative? And, lil3laiin3laiin, did you get everything cleared up with your positive TB test?

I am hoping that soon-to-be Plebes and parents have finished up all the paperwork and are looking forward to a relaxing June!! :thumb:
