So for the police record portion, what if I got in trouble as a 5th grader for putting a divot in the green of a golf course? I had to go to the police station and pay the golf course $50, but I don't think it was ever put on my record. I asked a family friend who is a cop and he looked at my record and said there was nothing on it (except a traffic violation). So do I have to report it??

If you were cited, arrested, convicted or fined then you must report it -- otherwise you do not need to report it. Note that the question didn't ask if it was on your record. If you are still unsure, then you need to contact admissions for clarification.
