PTR Question

    Good morning all,

    I am finished with my PTR packet except for two items which I cannot seem to figure out. On the checklist, there is a form called "Adult and Pediatric Immunization Questionnaire" which I cannot seem to find. Was it part of the green immunization form like the TB test was? There is a medical background form, too, which I cannot seem to find on the checklist. So I am not quite sure what to do with that either.

    Secondly, the checklist says something about a Religious Command Questionnaire that I am supposed to bring with me on I-Day, but I can't find that in the packet either. I know there was a letter about it, but I did not see a form or something to fill out.

    As we all know, the PTR has a lot of stuff in it, so hopefully my organization skills have not failed me and I have not misplaced something. Please let me know if you ran into similar issues, or if you can help!

    Check your email. The admissions department sent out emails correcting information about those two forms.

    Hmm...I did not see an email from admissions and I am pretty good about going through my emails.

    Do you know what they said about them?

    Congratulations on the acceptance of your offer of appointment to the United States Naval Academy! You should be receiving your Permit to Report packet in the mail shortly, if you have not already. This packet includes all of the forms necessary to induct you into the USNA Class of 2018.

    Please note your report time on your individual letter. This is the time you should report to Alumni Hall at the Naval Academy on July 1, 2014 for Induction Day.

    There is one correction to the Permit to Report Package Checklist. The Adult and Pediatric Immunization Questionnaire is not in the packet. It has been replaced by the Medical Record - Supplemental Medical Data (NHCANNA 6150/2).

    Please direct your Permit to Report questions to your Admissions Counselor listed on your online candidate application.

    There is also no religious questionnaire just the informational letter.

    I vaguely remember filling out something about religious preference ON I-day at one of the stations...however it has been almost two years so I could be thinking of something else.

    For '17 they had a forum that came with the PTR that we were suppose to fill out and bring with us on I-day, but if you didn't have it, the Chaplin's gave you a new one to fill out on I-day which takes only about 30 seconds, if that, to fill out. So maybe they're just doing that for everyone this year.
