PTR Question

    On the Medical record - supplemental medical data, a "requiring document" title and number are needed along with an issuance date. I have looked through all the papers and cannot find an obvious answer. Please help!!

    On DS's form, the title is supplied ("NHCA IMMUNIZATION GENERAL CONSENT (BMU and NHCA)"). This question also came up in the Facebook parents' group, where the answer provided by someone who called Admissions was "leave them blank." ICYMI, the Facebook group is called USNA Class of 2019 - Parents Page. There's some helpful info being shared there, too. It looks to me like this is for "catch-up" immunizations to be provided at I-Day or at USNA. And in the checklist it's noted as a "bring, don't mail." If anyone has another interpretation, please share!

    DS goes tomorrow for a polio booster and TB test, then we can check the immunization form off the list. It's oddly satisfying, knocking these off one by one...

    Yes, we called admissions and you should leave the two boxes at the top blank and complete the remainder.
