The "Record of Acceptance, Obligation, Reimbursement, and Oath of Allegiance" Form. It came with my BFE on Friday along with the Armed Forces Bank papers
 
Hi,
I am finding some of the requests regarding these forms unclear?
1)Immunization Record- it must be completed by the health professional (i.e. nurse or doctor) and faxed into the academy at 719-333-5448.
Then you must still bring a copy of this immunization record for I-Day.
2) You must have a copy of your eye glass prescription and must submit a copy to the USAFA at least 3 weeks before BCT. You CANNOT wear contact lenses at all during BCT.
3) You must complete Standard Form 86 questionnaire which is 127 pages long. This document must be signed in 4 places and brought to BCT for I-Day. I am not sure if they sign this document at I-Day or come with it already signed. Everyone also needs an original birth certificata, or passport etc.
4) Final school (transcript) grades must be sent to the USAFA as soon as school is done.
5) Form IMT-O-143 which is the Record of Emergency Data for Cadet Personnel must be completed and brought to I-Day. However, do NOT SIGN this form until I-Day when you and an officer will sign together.
6) SGLV 8286- which is the Serviceman's Group Life Insurance form which designates beneficiaries and the amount of life insurance you elect must be completed and sent in the the USAFA. However, I cannot tell if this form is signed first and sent or is it sent in without being signed. I have asked this question to the USAFA?
7) The cadet Sponsor program needs to be completed and signed ASAP.
8) The USAFA Assoc. of Graduates needs to be completed and signed ASAP.
9) The measurements for the uniforms need to be completed and sent in ASAP.
Anyone who can clarify what is signed in advance and what is not signed in advance- it would be very helpful.

On the Security clearance form: I emailed the Security NCO at the Academy. The appointees will be inputing all the information in a computer when they get there. There is not need to sign the documents now! We found this out after making a duplicate copy, now we have 250+ pages, but DS will never be at a loss for security clearance references.
 
Whatever you do, never lose that security copy. Clearances are only good for 5 yrs.. You do not want to start from A again because you lost or misplaced it.

Just out of curiosity, why such an extensive security form? It doesn't make any sense to me because the cadets do not need TS, and with attrition rate of at least 20%, that is a waste of man powers hours verifying the information, knowing hundreds will never commission. Worse yet, since it is only valid for 5 yrs., these same cadets will have to go through the TS as C1C's in 4 yrs anyway.

Maybe our ALOs, Flieger and CC can inform why the change.

The only thing I can think of is that cadets during summer do summer tours at Operational squadrons.

We had this exact situation 2 years ago. We simply stated in his paperwork that we were taking care of the debt. This worked fine and we did not have to pay it off all at once.

The security clearance form 86 is used for clearances from Confidential all the way up to TS. I filled one out for a Secret clearance when I started my job with the FAA even though I held a TS in the military. The Secret clearance is good for 10 years, which I have to fill out a much shorter form of the form 86. It is also 120+ pages long due to the fact that it gives the applicant a lot of repeated pages to fill if you have lots of relatives and lived in multiple places and held multiple jobs and visited multiple countries and knew multiple foreign nationals etc.

The appointees are also fortunate that they are so young that they don't have to go back too far. 30 years ago I believe that I had to go back almost to conception to get my TS when I went in as a nuclear weapons specialist.

You are correct to keep a copy of this form as it will come in handy in the future.

Required Documents

On p. 12 of the Instructions to Appointee Handbook is the page outlining Required Documents.

The first section, under "Name Change" it says to bring a photo ID (dr. license) and Social Security Card. I know this sounds like a dumb question, because it's under the name change section, but on page 25 where there is a checklist, there is also the photo ID requirement. So I have to conclude that all new cadets bring a photo ID. So is the SS card only for those with name changes? I'm only concerned because on the top of p. 12 it says DOCUMENTS CANNOT BE RETURNED, so I don't want to sent DS's SS card with him if we're not getting it back.

Also, under the Security Clearance section, for proof of citizenship, it says that the new cadet must hand carry one of the following original documents: birth certificate or passport. I don't really feel comfortable handing over my son's birth certificate if those documents "Cannot Be Returned." It seems unreasonable that the Academy should ask for the SSN and/or the birth certificate (or even the passport) and then say DOCUMENTS CANNOT BE RETURNED.

I can understand that they would require those documents for a security clearance, or another reason, but they should be returned. Is there something I'm not understanding here? Anyone know who/what office I should call at USAFA to get clarification? Can anyone give me clarity on how this was done in previous years?

DS did not receive this either. I am contemplating calling his admissions counselor to see if it was an oversight. He definitely needs glasses/contacts so this would be pretty important for him. Not that he wants those really cute glasses :shake:

I was worried last year too, YorkieMom. I ended up going to the Dept. of Health--Vital Records and got another copy of my son's birth certificate. The woman there said it would be considered original and official as it had the stamp/seal imprinted on it. It worked.

My son did take his social security card, but I vaguely remember seeing it on Parents Weekend. He had all the unused, extra copies of inprocessing paperwork locked in his drawer (which I took it home). Seems like I saw the card in there, and advised him to keep it locked up and not carry it in his wallet or anything. I'll have to ask him for sure, but I think he did get it back.

Edit:Just talked to my son (FB chat, really) and he did get his SS card back. Locked in his drawer for now!

She had recently gotten them and was curious while at the clinic and asked about them for BCT this year since her brother is in the class of 2016. The word at the time was that they still had plenty of the older frames and that they would use those up first. They figured they had an ample supply that would outfit the class of 2016. Might not see that change until summer of 2013.

Last year the answer was no, they had to wear AFA issued glasses. There is plenty of potential for glasses to get broken, esp. during BCT2 in Jacks Valley. Everyone else who needs glasses will be wearing them too. My son did say someone he knows broke the BCG's during Basic and they couldn't replace the prescription quick enough so that person wore his own glasses for part of the time.
 
Just because of that, and if I receive the honor of gaining an appointment, I will be wearing BCGs to I-Day (since we can't wear contacts until after BCT). And my shirt will be tucked in long before my first flight leaves en route to CO.
 
That might have been my son...his glasses broke before BCT II. The pictures of him on webguy were so funny; at first he used duct tape to hold the glasses together, then started using white medical tape. Eventually the frames wouldn't stay together so, he wore his own.

Will a passport and government-issued ID suffice if you don't plan on bringing your social security card and birth certificate

On the Memorandum for Cadet Appointee Class of 2016 regarding Security Clearance, it says that "To verify citizenship....bring an original of one of the following: birth certificate, passport, certificate of citizenship, or naturalization certificate." And then in the Instructions to Appointees, it says to bring "Photo ID" and "Security Clearance paperwork (Questionnaire and proof of citizenship)."

So to answer your questions, I believe YES, a passport and a gov't issued ID (that has a photo) would be enough.

And what about the social security card? Is it only required for those who got their names changed? Or do I have to bring along with me to in-processing? My parents are anxious about me bringing it since I can easily misplace or lose it.
