Questions about "Instructions to Appointees"

Okay thanks. raindeei any idea how many pairs would be good? I will also be getting to COS a couple of days early so Im assuming I should just leave whatever extra civilan clothes I have with the family im staying with?

I'll chime in, two pair if they let you keep them. You will not see the close on your back and feet until March next year!!!!!

Yea I only brought two pairs and that was enough. You will get the opportunity every week to send in your laundry.
 
For 9a and 9b... I just dont get what they're asking...:confused::redface:

I believe this is the Emergency Data form? This is the form where you give the AFA names to contact if you are injured or killed, and list beneficiaries for death benefits. You listed your parents names on line 7 and 8. If one of them is so ill that you wouldn't want them contacted directly if something happened to you, you would list that parent's name on line 9a and an alternate contact (another relative, close family friend, etc.) on 9b. For most incoming cadets, I would think these lines would be blank or say "Not applicable".

Awesome! Thanks! And another problem/issue, I downloaded the form from the other thread for the IMT O form, and at the veeerrrry bottom, it's dated "26 June 08" *sigh* I was doing great on the form they gave us until I wrote my dad's last name before his middle name.:frown: What do I do now? Lol

I actually messed up on my original form and this was before someone posted the form again so what I ended up doing is scanning it and erasing everything (with photoshop or Paint) and then printed it. I could probably send you my blank copy if you would like- send me a PM.

I think you're referring to the form I posted. I'm sorry I forgot to address the date issue and the bottom. My guess is that it is supposed to be dated for I-day this year. I suggest printing the form, whiting out the date and making a photocopy. That way you have a nice, clean sheet of paper and you can write your own date

Yea I actually just compared the forms and the form we received in the mail doesn't even have a typed on date so the method above is fine.
 
Does anyone know if it is true that we are required to submit our AP test scores for validation? I have heard that we have to, but some people have also told me that validating scores at a service academy just makes life more difficult during the Academic year. Any thoughts?
 
Validating classes is questionable. They will take them into consideration and if you qualify you will be put into the next level class. As for making it harder, not necessarily. If you earned 4-5's and based on your placement tests, they can put you up a level. This in the long run can open time in your schedule, example: my DD had 5' in two AP English classes and two AP History classes. She did validate English and History in a way. She was allowed to take a higher level class in English that she found more interesting and was able to do a similar thing in History. There are always required classes, but she will have more flexibility as a senior.....this is what she told me. Based on her first semester grades her C4C year she was also invited into the Honors Program at USAFA. She enjoys the environment in these Honors Programs classes. Much of it depends on what works for you.

For the immunization forms, don't bother trying to email the immunization record. Both email addresses don't work. The Allergy & Immunization Clinic webpage doesn't work either. If you want to verify that the A&I Clinic received the fax copy of your immunization record, their phone number is (719) 333-5080.

Thank you for posting the phone number. I sent three e-mails to the immunization clinic which all came back and now you saved me some legwork!
 
Paperwork

Okay, we have seen all of the paperwork that is being discussed on this forum, and DS and I have a question about this.
So far, he only has the uniform information to complete and the immunization record to fax back. What other paperwork is out there?
Any information would be appreciated.

You have probably not received the second BFE. There is paperwork regarding Security Clearance, Emergency Data Collection, Life Insurance, AOG Bed and Breakfast(optional), AOG Webguy, Cadet Drum and Bugle tryouts(optional), Sponsor package for the cadet, and possibly banking information for Armed Forces Bank (I think that was part of the first BFE though). There is also a booklet, Instructions to Appointees, it has your post office box number on the front. Depending on when you received notification of your appointment it should follow soon.
 
What about the tattoo policy disclosure form that needs to be signed off by your ALO?

Okay, so I missed one or two. I never stated that it was an all inclusive list. The tattoo form will also be in the second BFE. There may be others I missed too!!

Thanks for the info. DS hasn't received the second BFE yet; so that's why we aren't familiar with all of the paperwork. Hopefully, he will be receiving it soon!

No need to apologize, I was half kidding. I think that the tattoo form was kind of odd. My DD had to make a special trip to meet with her ALO, go over the policy in person, sign and initial the form in three places. Couldn't explain the policy over the phone...a face to face meeting was required and the ALO is about an hour away. No visual exam was required but you do have to sketch out your tattoos on a cartoon outline of a human body. Has there been a problem in the past with appointees showing up on I-day all inked up? It is pretty apparent that plenty of football players have them but I am sure they are following the 25% policy.
