On the instructions sent in the mail it says to enter your appointment dates online, but I could not find an option on the website?
What kind of information is required when explaining about X-Rays in the history form?
What purpose do we list in form 2492?

1. Click home on DoDMETS. On the left side of the page there is a toolbar. 4th option is "Schedule Exams." Click this.

2. Not 100% sure on this. So I won't answer.

3. If you mean the "purpose" section right next to where you fill your name in...I left it blank and had my examiner fill that out.

How important is it that I have the physician's info for when I disjointed my pinky finger? It happened 6 years ago in a different state and it may be difficult to find the information.
