Greetings,
My son is filling out his DoDMERB medical history and we are having problems locating our records.
These are the two medical situations:
Age 2-5 Tonsilitis and ear infections (2 x per year) We have the name of the clinic in Dubai but not the Dr. Name or EXACT dates. Treatment was antibiotics. Treatment in South Korea was the same, but no doctor.
Age 3 Hairline fracture of Tibia. We have the name of the clinic in Dubai but not the Dr. Name or EXACT date. We do know what the treatment was. Cast 3 wks, 2nd cast 3wks.
Age 5 Tonsils and Adenoids removed in California, but we don't know the doctor's name or EXACT date. I'm searching!!
After that, he's been healthy except for a case of Mono last year.
1. Do I need to worry about not having the entire history?
2. Do I input Mono under "other illnesses"
Any response will be greatly appreciated. He goes next week for his exam.
