Records Needed for Administrative Remedial

DS is currently going through administrative remedial for a few issues. I have all necessary medical records, except for one. DS had a shoulder injury about a year and a half ago (nothing broken or dislocated) and the doctor sent him to PT for about a month. I have all the records from the doctor (both before and after PT), but I don't have the records from the therapist. Since the doctor released him with no limits on activities after the PT, do you think the records from the doctor are sufficient or should I take the time to get the records from the therapist?
 
my memory of a similar records chase with my DS is that the Drs records included the PT (and other) reports. A phone call to the Dr's records person should clear it up. Likewise, the PT office will likely be able to print or copy records in a short time.

I guess I'll start with the therapist's office on Monday and see if I can get something from them quickly. When I went through the process of getting the records from the medical doctor, it was a bit of a pain (not getting back with me, etc.). The records they sent over didn't include anything from the therapist (which I found a bit odd).

We had to collect individual records from each doctor and the physical therapist for our DS. I called ahead and told his story and had great cooperation from the different staffs. And our doctors did not have PT reports or records. If anything, they received status reports from the PT but not detailed records. Hope this helps.
