Remedial confusion?

I fractured my left ankle about 2 years ago, and Dodmerb has requested a remedial for it. They asked me "DOES NOT REQUIRE A PHYSICIAN'S APPOINTMENT: Please complete the ATTACHED ORTHOPEDIC QUESTIONNAIRE REGARDING left ankle fracture" I see no attached questionnaire? did they make a mistake of not attaching the questionnaire or something cause all I see is directions and no questions?
