Revoked Appointment?

Aside from getting in trouble with the law, what are other reasons appointments can be revoked? Injuries since dodmerb exam, final grades, etc.? How often are appointments revoked for grades and other reasons? I'm not worried, I'm just asking out of curiosity.
 
As a general rule, the main reason appointments are "revoked" is a late medical issue -- typically a serious injury that would prevent you from participating in PS. For example, a broken bone.

Grades generally are not an issue unless your grades drop precipitously without reason causing USNA to think you "blew off" senior year after receiving your appointment. So, if you were getting mostly As and Bs and suddenly are getting Cs and Ds across the board, that could be a problem. Legit issues might be, for example, death of a parent or sibling.
