ROTC interview

I googled a form for the interview, ill put in the link, but I am confused on what exactly the difference between these two parts of the athletic section.
-Either active involvement in organized competitive (club, church league) team, sports or active
-Membership of regional/city/competitive league

I wrestle for a my high school team and I wrestle for a local club so that should cover the first one, but the 2nd one also sounds like a club would cover that to. I just don't get the difference between the two.
I also have played football and lacrosse.

Also, if you know any questions they may ask or will ask during the interview I would love to to know.
