Searching for AF IMT 2519 V5

Can anyone provide me with a copy of or a link to the AF IMT 2519 V5 (Field Training Uniform and Equipment Checklist)? I know there's a checklist in the FTM, but I was asked to locate the source document.

I've tried ePubs and Uncle Google.
