Sending required forms

Should the forms that are at the back of the Instructions For Applicants Offered Admission download be sent certified mail return receipt requested or will the gray envelopes work? I'm concerned about them getting lost.

Gray envelopes work; you can't do both certified and the gray envelopes, so I just used the gray one.

Quick question on the forms. I am deployed and I do not get my mail directly. My parents have told me they received the appointment folder and some gray envelopes. However, they said there weren't any of the forms described in the Instructions For Candidates Offered Admission pdf other than the appointment acceptance card, which I already filled out. My portal shows the four online forms, such as travel, etc which I already filled out. Should they have gotten the medical form, background check, dental, ETC in the mail? Are we waiting for them to be mailed or are they online?

The forms are in the back of the "Instructions for Applicants Offered Admission".

After page 17 you will find the forms for:
- Birth Certificate Information
- Certificate of Authorization
- Statement of Consent
- Request for Final Transcript
- Vision Survey
- Immunization Record Form
- Police Record Check and
- Appointment Declination

Ah indeed they are now. The PDF was only 17 pages when I first got it, now it's 27, hence my confusion.
