Does anybody have a list of the required vaccines? I only selectively vaccinate so I know DD will need some to get caught up and I'd like to start now and space them out as much as possible. Thanks!
 
When you are appointed and then accept, you will get a set of forms to fill out. This used to be in the 'BFE' but I believe that many of the forms will be on the portal now. My DS needed a total of 5 shots (and he was up to date) which included a tb test. He even needed a polio vaccine although not 'medically necessary' according to pediatrician, USAFA required it (had to be IPV, shot, not OPV, oral). VERY IMPORTANT that you have your DS/DD carry a copy of the vaccines with them to I Day, otherwise they will just get them all there and those arms will be sore for BCT!
 
She is appointed and has accepted but all she's received so far is the appointment letter on nice parchment paper. No bfe, no other forms.

Great information. Appointees absolutely should carry their shot records on them. They will submit them before I-Day but if the records are lost/misplaced and they do not have a copy on them, they will receive all vaccinations again as UsafaMom so accurately pointed out. Leaving the records in their backpack on I-Day will not help them either. They should carry an extra copy on their person as they will have sufficient stress on I-Day without extra shots.

As far as paperwork is concerned, Appointees should keep a hard copy of everything they submit once they begin receiving forms to fill out. And the forms will come, perhaps later rather than sooner and I suspect to their online portal. DS took a copy of all his forms including the rather large security clearance form in an accordion style folder so that he could quickly find any forms that were requested of him.

Necessary, and better than arm's full of shots to be sure!

The vaccination form should arrive in the appointee packet that'll arrive shortly following the acceptance letter. Make a copy of the blank, make a copy of the completed form, and get it done ASAP (some of the vaccines require boosters, which are much better to get done during BCT than the school year, believe me). Also, ditto on the advice to carry a copy of the form - carry copies of everything. Form 98, vaccines, clothing fitting, travel orders (make three), itinerary... if you filled it out, make a copy for home, make a copy to carry in a folder, and make a spare to put in the backpack.
