**** REQUIRED ADMINISTRATIVE AMI ****

R259.10 --- We have determined that you do not meet the DoD medical standards for the following reason(s):

D155.80 - Astigmatism in excess of 3.00 diopters

D231.10 - History of a depressive disorders

D232.11 - History of obsessive-compulsive or related disorder(s)

D232.80 - History of other mental disorders . Your file will require a review by the medical Waiver Authority of the Service Academy(ies), ROTC program(s), or other program(s) to which you have applied. However, before the waiver process can be initiated, you must complete the AMI(s) listed below: After we receive and review your AMI(s), we will send your entire file to the Waiver Authority for medical waiver consideration, based on their individual processes. Additionally, after receipt of the requested AMI(s), we will send you a letter that will explain the waiver process for each program. PLEASE REVIEW THE REQUESTED AMI(s) BELOW CAREFULLY AS THEY MAY BE EITHER "MEDICAL", "ADMINISTRATIVE", OR BOTH AND FOLLOW THE SPECIFIC INSTRUCTIONS FOR COMPLETION.

Did your email require medical tests to be done?

If yes,

check your DODMETS portal.

If it is an administrative AMI, it will require a personal statement from you about your condition, and all you need to do is to state exactly what it asks you to and then wet sign the document. If it is a medical AMI, check your DODMETS to schedule an appointment or schedule with your own provider ( I recommend your provider because they know your medical history better). Make sure to notify whoever you see exactly what the AMI asks for.
