Last year, the earliest you could start your SLS application was December 15th.
You will fill-in a form with information such as your name, date of birth, SSN, address, school you attend... Also, you will put in your PSAT/SAT/ACT scores and some athletic participation information (Varsity Letters, Captainship) and other Extracurricular Activities (including Class President, Student Body Officer, Boy/Girl Scout...). That's the pre-candidate questionnaire.
After filling that in, you will be able to "apply for SLS". You will get to order which workshops you wish to attend (basic introductory classes in a given subject - I personally recommend Civil and Mechanical Engineering Workshop). For SLS 2011, we got to attend 3 workshops (normally the highest in order. I haven't heard of anyone who did not get their 3 prefered classes) out of the 15 available.
You will get a letter notifying you have been accepted. You will have to fill-in a few papers (such as a Power of Attorney letter if I recall correctly, and a Medical Form).
PRACTICE FOR THE CFA! You will get a chance to take it in SLS. There is no reason why not do it and pass it.

And hey, when you get there, make sure you enjoy it
A lot of people call it "the best week of their lives".
