What do I send in to accept appointment?

I just got by bfe a couple of days ago, but I was wondering what I have to send back? I know that I have to send back the form that has 2 other copies attached to it, but do I also send in the sheet that has the oath of allegiance on it as well?

Send back what requires signatures. P.s. always make copies of anything you send them.
