Default Statement of Physical Actvity for the past year

Well I was able to get in to see the Md's and everything is fine however I had a shoulder surgery back in 07'. I have all my records regarding the injury and subsequent treatment. Now I am ready to send my records in for further evaluation. I am also supposed to send a 'detailed' STATEMENT OF PHYSICAL ACTVITY FOR THE PAST YEAR. Does anyone know what format they are looking for? Should this be in paragraph form or bullet list? Additionally, should the activities be listed oldest to present or vis-versa? Lastly, my OSO said the MARINES have a 'shoulder specialist' that needs to evaluate my shoulder as well. Should I wait and send my stuff in until after I have seen the ortho?

My son submitted a detailed statement of physical activity in a letter format with information regarding all of his activities. There were no specifications as to the format required. His was in paragraph form with details regarding type of activity, duration, frequency, and difficulty. As far as the timeline, he wrote his detail from the beginning of the post operative period to the present.

On your last question, so far my DS has not been directed to see a Marines shoulder specialist. Although I believe that his paperwork and records have gone before the Waiver Authority ortho specialist (Marines). I don't know where you are in the process, but once he was DQ'd his record went before the Waiver Authority. The Waiver Authority asked for more information, namely they wanted a full evaluation no sooner than one year post operative, including imaging. That will be in February. So in February he will submit this information and the process will continue to move forward.
