What is the Candidate Activities Record part of the application? Clicking on it send me to a page exactly like the school references, it asks for a name, email, etc. What will the person be filling out, so I know what name I need to put down?

I just completed this part.
First you fill-out the school official that will verify the activities. When you submit that, another window appears which is the activities page. You fill this out and submit it. The school official will then get that product to verify.

Such as extracurricular activities?
 
It's usually your counselor that fills out the form and verifies your activities.
 
Sorry but where can i find the CAR? I can't seem to find it. And is the Candidate Personal Data Record only 2-4 pages long? And where do I find the SOE forms? Sorry to bother you.
 
The CAR and SOE "forms" are actually emailed to the teachers (SOE) and whoever you designate to verify activities/sports/varsity letters listed on the application - only the individuals you identify receive the "form", which is an email, fill in the blank sort of thing.
 
should the person who fills out your CAR be the same person that fills out your academic information?
