I just read over the Instructions for Candidates PDF again and noticed that the immunization form is due 15 April. I did not realize this until now and I hope to schedule a doctor appointment as soon as possible to avoid turning the form in late. Will Admissions make a big deal out of it if they receive the form past 15 April? I should have seen the date before, it's easy to miss in that huge packet of information.

Well, the NE Admissions Officer sent an e-mail to every 2016 candidate, and he said one of the things he DOES NOT TOLERATE are delays due to lack of preparedness.

Try getting that doctor! NOW!

Sorry can't be any more useful, just thought I would put this out there. Remember, I am a 2016 candidate, so I have no relation or information to the same level as you do.

What if I haven't even accepted my appointment yet? Do I still need to have the immunization form done by April 15 if I have till May 1 to decide?

Well, you should call your Admissions Officer (primary point of contact) I suppose. But try working on that immunization ASAP anyway. Read my Sun Tzu signature ;)
 
Call your doctor and get it done ASAP

If your doctor ends up being as screwy as my son's, you might be hard pressed to get it in. It took 4 separate visits over a one week period for those losers to figure out the immunization form and fill it out. My wife finally had to go down there and do it for them.

Unbelieveable.
 
My daughter went to get everything taken care of in February. Immunizations needed to be updated and TB test done. We are still waiting for the doctor's office to get everything filled out properly. I have called numerous times and always something they are waiting for. They had to get some of her files from off site, etc....

Now...here we are up against the deadline.

It is the last red sitting on her portal. We will be emailing her WP contact if DR. doesn't come through soon.
 
I thought it said to have immunizations before R-Day, but I guess i am wrong
 
Get it done ASAP..my son had a positive Mantoux (TB test) and we had to wait for the blood test to be done, was just an allergic reaction, but it delayed the form getting sent in by a week. Definately let admissions know if it is not going to get there before the 15th. at least fax/scan a copy till the original can get there.
 
Thanks for the advice. I contacted Admissions and told them the truth - that I had overlooked the due date of the form and am working to get the form filled out as quickly as possible. I asked if I could fax the form to the number listed at the top of the Immunization Form cover page as well. Hopefully they won't be too critical of my mistake, I'm so busy all the time it's easy to put off paperwork for something that's months away.
 
Our experience with our regular doctor was very much like what I am reading here, they just did not seem to know how to handle this form. Lucky for us as DS and I were driving home one day we past the usual Doc-In-Box clinic we use all the time for minor medical care and I thought, why not see if they can help. They completed the form in an hour, all he had to do was come back to have the TB read and we were done. Difference is, the minor emergency clinics do a lot of occupational medicine type of work (which is what this exercise is) and your usual General practioner and his/her office don't. Hope this helps, it was a God-Send for us. Go Army!

The Admissions Support Clerk said to fax the form to (845) 938-3021 Attn: Pat Adams (Not sure if the name Pat Adams applies to all candidates however, as Pat may be the Admissions Support Clerk for my region, state, etc.). The clerk made no comment on what would happen if the document was late. If I happen to not be able to send the form in completed by that time, I'm going to fax the incomplete version just so they have some information in the system.
 
I am also in a similar boat. I didn't wait until the last minute, it just took a month to get an appointment with my doctor. The when I showed up at the doctor, it was like the form was in a different language. Overall, and after several phone calls to the head nurse, they realized how to fill the thing out.

I will have the form done on thurday when I return for my results, but unfortunately, US Mail won't be getting my forms to West Point in one day. I am hoping that I can fax the form before the deadline and mail it. I guess I will give them another call to confirm on Monday.
 
I see no April 15 deadline on my Portal. In fact many people with acceptances will not commit until the deadline (May 1). Others will not get final appointments until after there are some declinations. MANY people will not have the form in by April 15. Where did you get the info about a deadline?

In order to speed up the process of immunizations on R-Day, this form must be filled out and returned to West Point prior to April 15. (Late appointees should return the form as soon as possible.)
 
I realize I should have seen the date in the Candidate Instructions booklet before, but USMA admissions really should add dates to the forms checklist on the portal. I feel like a procrastinator because I did not carefully read all the information on the forms the first time I read through the packet of information.
 
getting the blood type lab work was a problem

My daughter now has her immunization form filled out. We have just had issues with getting her blood work done to get a blood type report.

The doctor wasn't sure how to handle this. She felt like she needed a 'reason' put on the orders for the blood work. She put down that my daughter needed her iron checked.

So we now have our lab orders and will be going on Monday to get that lab work done. I am not sure how long those results will take.
 
I don't think everyone should stress out over this form. Yes, it is a pain to get anything through any doctor. It took us 2.5 weeks to complete. I ended up taking my son to a walk-in clinic for the adult polio booster. His pediatrician, had a hard time understanding the need for a 5th booster. The key part I read was to make sure that you are up to date with all of the required items prior to RDay, so you won't need to get the shots on your 1st day and possibly have a adverse reaction. Don't forget you will need to carry a copy of this form with you when you report.
