Where are we suppose to send the Life Insurance form that came in the Instructions to the Appointee packet? I cannot for the life of me find where we are suppose to send it or if we are suppose to sign it. Thanks!

I just threw mine in the main envelope with a bunch of the other stuff and the other insurance form (the one you print off).

I'm pretty sure that's the one you need a witness to sign so I'd leave the signature blank. The instructions sheet says something along those lines.
 
Thanks! I feel like this is one of those forms that we should send in but also have a copy with us to sign on I-Day. I will send it into the main address in a =nother envelope because I already sent the rest of the stuff off.

Just a warning, I believe I recall the instructions stating that you should not hand carry the forms with you during indoc.
 
The life insurance forms DO NOT get signed. That was in bold letters at the bottom of the directions page. After completing the other parts of the form, then it was to be mailed in the provided envelope to Cadet Personnel. But, as has been pointed out on other threads, taking a copy with you of all submitted paperwork is probably a good idea. Things do get lost and with 1000+ incoming cadets, something will. You don't want to slow down your in-processing if you can help it.
 
Unless things have changed dramatically in the last 2 years -- YOU SHOULD HAND CARRY A COPY OF ALL YOUR PAPERWORK WITH YOU TO INPROCESSING. Especially your Immunization form. That way if your paperwork somehow did not reach the correct file, you won't be re-given all the immunizations that you have already had. Put all important paperwork in a zip lock bag so that it is easy to carry with you and easy to find. Just a parent's suggestion -- definitely mail all original required forms, etc. as directed in the instructions but make a copy of everything before you send it in. Leave one copy with your parents and take one copy with you.
