Whats on the AF form 4060?
A complete evaluation of several categories of leadership potential, background, fitness, etc. It's a "Confidential" form and a completed one isn't for general view and the scoring system is also "Confidential."
That being said, it asks questions about:
A. CHARACTER/CORE VALUES
B. SELF-CONFIDENCE
C. HUMAN RELATIONS
D. PLANNING AND ORGANIZING
E. COMMUNICATIVE SKILLS
F. LEADERSHIP
G. MOTIVATION TOWARD AIR FORCE
H. MY RECOMMENDATION FOR SELECTION
And then there are several "specified" areas that we ALO's write our "overall assessment of the candidate." We get 50 lines total. We're asked all sorts of "subjective" things like: "What would the candidate look like in a uniform?" (think bearing, professional image, fitness, etc.) just as an example.
If your ALO hasn't told you that the 4060 is ALL ABOUT YOU!!! Then you've missed the message. This is YOU wrapped up into several blocks and then 50 lines maximum.
Which is why I say: "Write your I love ME" paper early and often, updating it all the time for the ALO...all the things you've accomplished, nothing too small to mention." Not only does it aid we ALO's who are short on memory space, but it's a great way for YOU to remember things you might have deemed "trivial."
NOTHING IS TRIVIAL when you're competing at this level!!!
I could go on and on but that's basically what the 4060 is.
