Are mids given 4 years only to finish? What happens if you fail a class? Are you out? Make it up? Are there any that complete four years of academy that don't graduate (not asked/made to leave, just don't have the academics for a bachelors)?
What happens to those that don't graduate?

yes. but there are exceptions with everything. failing a class= mando summer school. im not sure if i get what your asking for the last part. if by "don't have the academics" you mean don't have good enough grades/are failing, then they make you leave.

If you're academically unsat (less than 2.0 GPA, 2 Ds or 1 F) you get what's called "ac boarded." Ac boards suck. You basically have to try and find a way to prove to the board that you deserve to remain a midshipman and should get another chance academically. You can retake classes, either the following semester or during summer school. Every year there's a very small number (like ones and twos) who get delayed graduation for academic reasons. Usually these are athletes who for various reasons (time commitments plus bad grades) couldn't complete the academic program and they get one more semester to finish up.
They're more forgiving to plebes and youngsters at ac boards, but once you hit 2/C year and have signed your committment papers, you really need to step your game up.
The Academy is fully willing to kick kids out after their final finals. As in, after their last tests ever. Every year there's a few who get told that they didn't satisfactorily complete the academic program and so can't commission. They're left with a big debt to pay off and nothing to show for it after four years.

Hurricane12 nailed it. Every year there are a few. We had a companymate who is currently a Petty Officer instead of a LtCDR because he did so bad his last semester. He had a pilot spot too. The Academy recognizes that some Mids miss the mark, but like everything else they look at the whole person when making decisions. Did this person fail because of lack of effort and preparation or did they work their butt off and get alot of extra help, pay attention, complete all the course work and work with their professor? Is this a good Midshipmen who will make a good officer? Every year there is a graduation ceremony held after summer school for usually 3-5 people and again another one in Decemeber for even a smaller number. So it does happen, just in very very small numbers.
