SLS Offered question

For USMA "SLS Offered" candidates, do you print off the PDF file on the left of your profile SLS tab and send it or should I be expecting a package to come in the mail first? There was no email or mail that notified me of the offer.

From the answers I got on the acceptance thread, we are probably going to get snail mail that tells us when we have to send it in by. But look at the PDF, there are a few things you need someone else for (physician's OK, and a legal form that has to be signed by and by your parents in the presence of a 'notary public').

Yeah, I have it printed and filled out already. Had my physician sign it and had it notarized. I'm just not sure as to mail it in or wait on the snail mail.
