Does anyone know about this form? I appeared on the portal along with other forms.
Employer's Evaluation of Candidate - USMA Form 5-518
Not sure what to do with it.. Since I've never had a J.O.B. it doesn't apply to me. But I'm a little worried about leaving anything unfilled..

Also do I need to re-take the CFA if I already took it at SLS this past summer?

If you have never had a job, leave it blank or type "Non-applicable". I imagine you will need to take an updated CFA, but that may be a question for your USMA Rep.

I would ask your RC to re-confirm. However, you should see the "Green Check" next to the CFA test you took during SLS. I don't think the Employer's Eval. Form is new...it was there for those who needed to submit as part of the original applicaiton. Since you have an Appointment, I think your CFA should be up to date. Now, just need to make sure you submit Transcripts and all other documents in "Instructions For Candidates Offered Admission".
