Late Paper Work?

What would happen if I do not get my Air Force tattoo form(Form 4428) in before the June 8th deadline?
 
You still have time to FAX it in and get it there on time. Is there another reason, say getting a tattoo now, that is preventing you from sending it in? At the least it will delay your in-processing, and at most they could turn you away on I-day and not reimburse you for the return trip home.
 
It is just because I must have my ALO sign off on it, and I can't seem to get a hold of him. I do not actually have any tattoos.

Would you happen to also know the number by which to fax it in?
