Hospital unable to find records (Remedial)

DoDMERB has requested that I send in all medical records for a surgery I had when I was three months old. The issue is that the hospital can't/is having trouble finding the records and they don't know when/if they will be able to find them. I would hate to be DQ'ed for not submitting documents in time, what would be the best thing to do?

I had the same problem. I saw a specialist when I was three years old and his office did not keep records past 10 years. I got a remedial for the issue asking for all the records. My former pediatrician, fortunately had notes that I had seen this doctor and they sent me those. I sent the notes along with a cover letter explaining my efforts (actually my mom's efforts since they wouldn't even talk to me - I'm 17) to get my records as my response to DODMERB and included a note from my current doctor that I did not have any problems in that area any more. It must have satisfied them since the remedial was removed.

I would imagine this happens on occasion - if you cannot find the records or they no longer exist, I would call DODMERB at 800-841-2706 and let them know.
