^^^^First off, please read the guide that is in place on the DoDMERB website, paying particular note to the highlighted green section.^^^^
I recommend including your disgression as follows:
1) Look at the questions and see which ones pertain to drugs/alcohol.
2) In a Word document, create first a header with your Name, SSN and Date of Birth (ensure it is correct, as SSN is used to tie your application to you, and not someone else with your same name), and "DoDMERB Health History Answers Revision", all on 3 separate lines.
3) Below that heading, write the numbers for the answers you will be discussing (so, if questions 34, 35, and 36 are about marijuana and alcohol, write "34-36") do this for each question that you are changing the answer to (every question is about a different type of drug/alcohol, so label your responses accordingly).
4) Type up the CORRECT answers for each question (do not hold any information back about duration/time of use/frequency, etc.)
5) Create a line for your signature, a witness's signature, likely your parent(s), and a space for the date
6) Print this page out 3 times, and SIGN AND DATE EACH ONE with a PEN.
7) CALL DoDMERB directly, stating that you have such revisions to make, but only CALL after you fully type out your responses
....follow the procedure (roughly) for changes to your medical status (i.e. injury after exam, etc.). Be ready to
fax the information at soonest availability
This is the most professional way to note changes/corrections, and have them still be medically/legally binding.
Did you disclose this information on your applications? If you didn't, then it is no longer an error on your parents. That would be you not being honest with yourself.
Regards,
DS of Account Holder, USNA Class of 2020 appointment recipient
P.S. Withholding information from your parents will not make the drug/alcohol use go away. Denial, whether willful or by misinformation, will not be helpful
