SLE Medical Release Form

As I fill out the necessary paperwork to participate in SLE this summer, I have come across the form that must be filled out by my physician. Will having my doctor answer "yes" for having "had a medical condition requiring occasional medical attention" somehow rescind my acceptance into the program? It is honestly a very minor condition, and I have already discussed with my physician how to explicitly detail out in the explanation that the condition does not impede on my physical capabilities in any way, shape, or form. Any feedback would be greatly appreciated. Thank you in advance.
 
What is the condition? Things like asthma are disqualifierers for SA's.
I do not feel comfortable saying what condition it is, but I can say that I have contacted a very dodmerb knowledgable physician about the condition, and it is something that is usually met with a waiver. I say usually because I know that there's no telling what could happen with the dodmerb process.
But, all in all, it is nothing even close to asthma, diabetes, or any other condition that is an automatic DQ.
 
Then you should be fine! If you won't require constant medical attention, I wouldn't worry too much about. Make note of any medication you need and if they ask questions just be honest and explain what it is. Nothing to get too worked up about, they want to give lots of students opportunities.

Okay, thank you so very much. Having even this slight hiccup in my file worries me a bit, but I'm hoping it won't pose an issue come real application time. I appreciate the help!
 
@paperplaneenthusiast, you need to list your conditions. SLE attendees are minors and if West Point is accepting you into their care/custody they become responsible for your health. If you don't disclose something, they can't take precautions (having the right specialists, supplies, controls) in place to avoid/mitigate that condition.

No worries, I will be listing my condition on the form! I meant that I did not feel comfortable disclosing my medical information on an anonymous online forum. That is all.
 
I have a,question, the medical form for SLE ask for your weight?, my DD is very fitt and sporty but still need to lose some weight for Academy standars (she is,working on that now), and will be in that weight by June, but if SLE require her weight now, she will not fitt the standarsd for the Academy, did somebody know aboyt this?, thank you.
