Medical Forms for c/o 2020

The instructions says that this completed form is due today. However, what if we need to wait until we are 18 to get certain shots. Some of the vaccinations says we need to get after 18. So should we send the incomplete form? Also I have test results that I need to put on there that I can't get until next week.

If anyone can clarify that would be appreciated.
 
Don't worry buddy. The due dates aren't hard dates (for the forms at least). The FB page was clogged with people worrying about the deadlines. Debra Dalton just said to get them in ASAP. If you're not 18 then you won't need those shots. If you're missing shots, you'll get them on R-Day (highly recommend not to wait till R-Day..for obvious reasons). I sent in my immunizations a couple of days ago, and mailed most of my other documents in today. Don't stress out..but hurry up, lol PM me if you like

Thanks, I was planning on just sending everything that I have so far and then when everything is completed, either send it or bring it with my on R-day. I have one more question.

Is anyone having trouble faxing the vision survey form, mine would not go through. I used the number from the instructions packet and then I actually called and they gave me another number that didn't go through. How did you guys submit it?

Also what is this FB page that you spoke of?

You hand it in just with those spots back. They update your file and you'll have a green check and they'll send you an email saying that once you get your 18 year old shots your immunization records will be complete. If for some reason you don't get your 18 year old shots, they will be given to you on r day

Usma 2020 (official) that's the page. There's a lot of info there. Someone from admissions is in charge of the page, so you can ask her questions and she'll be glad to respond. (Names Debra Dalton). Only accepted candidates have access to this closed group (their parents also have a closed page..I recommend your parents check it out "usma 2020 parents page" )

In regards to the vision survey...I also had issues trying to fax it..so I just mailed it with everything else today.
