A required document to be included as part of the acceptance of the appointment to the Class of 2013 is a "Certified Social Security Card". Looked on the Social Security Administation's web-site and the closest I could find is a replacement card. Any other ideas would help.

Ours said the same thing...I called; they said a photocopy of the card would be fine. This was for USMA...I suggest you call and ask.

I got a DD's Social Security card and original of her birth certificate, and then went to the bank , had them make a copy and then NOTARIZE it...

and on the copies they wrote "certified as a true copy of the original"
the notary stamped it, signed it and dated it

SSA does not issue certified copies. Admissions says that a notarized copy will work.

This will work in some states. In others (like CA), you cannot legally notarize a copy. There is some kind of work around though. If you go down to your city clerk's office, they will be able to figure it out.

When we had to submit these last year, I took them to AAA and had them copy them and notarize that it was a legit copy. That was good enough. Just remember to make copies of EVERYTHING you send to USCGA and then bring a set of everything with you to R-Day. EVERYTHING!! (immunization/ titer results, sizes, etc). I also kept a copy back at home in a file in case my daughter lost them. We didn't end up needing it, but were came prepared (had heard horror stories of kids that had to get all their shots again because they didn't have the records). I also talked to someone up there in Health Services to make sure all her shots/ titers were OK.

Grappler's Dad - what part of PA are you from?

I see a potential travelling party! We are from Harrisburg & another MMI scholar is from Reading. Delaware area parents association will be having a party in June -- maybe we will see you there. We couldn't go last year because it was the same day as our daughter's graduation party, but we hope to go this year.

Sorry I researched this already.... :) and although not legel in all states it is legal in Washington (where I am at)... Funny thing is it is NOT legal in CT.. where the CGA is at... odd eh???

iwas wondering about the desinignation of beneficaries page. On the back there is something for annual "re certification". Do we have to sign this now or will we be doing that in a year from the date on the front? Thanks

Section V should not be completed now...as titled, it's where (as a cadet) you'll periodically certify the information is still valid.

Maybe it's a reflection on the "training" that notaries receive, but I get certified copies by notary all the time here in the Nutmeg state!

LOL THERE IS NO training given to Notary Publics.. it costs 30 bucks to the states, and 50 bucks for a 10k bond..... That'll get you the appointment, certificate, then you have to buy the stamp.

LOL... I went to Navy Fed and they told me they could not notarize copies, since they didn't have a stamp with all the information ?!?! SO I drove to Harborstone Credit Union and had them do it, but the girl there didn't know how to do it either :frown: ..

PS I am a notary public, and I can make copies and notarize other peoples "stuff" ... I can not notarize my own stuff so I had to take it elsewhere..

Law offices and city & county offices always have at least one notary but they usually charge $$ for the service. There are social security offices that you can usually go to for the SS Card. Banks and CU's usually do it free for their customers...

Another Question for Objee

For whatever reason (I forgot) we sent an original Consular Report of Birth Abroad certificate to admissions a copy of years ago. Could you be so kind to tell me if the cadet will receive it back at outprocessing, it would take a act of congress to get a replacement. Thanks

I just got off the phone with Admissions and a SS card photo copy that is notorized is acceptable but the Birth Certificate must come from the vital records department of the state of birth.

I appericiate everyones help with my question.

you are right about the act of congress on that.. I have one original and it says protect it with your life.... (almost) as it has a important file number on it.

Appointees should also get their "Appointment Certificate Presentation Request" form filled out and returned to the Academy as soon as possible if they wish to have their appointment presented to them at their high school awards ceremony.

The cadet should be able to just ask for and get it back now if Cadet Administration actually has it (they should) in the cadet's record.

Do CGA scholars get this too???
