I found out two weeks ago that I needed to fill out a remedial form. The isotretinoin/accutane questionnaire.

It needs to be signed by my Dermotoligist, but she's been on vacation the last 2 weeks, and now I found out today that she has just taken sick leave for who knows how long so she can't sign the form.

Am I done for? When does this form need to be completed?
Will this ruin my chances of getting an appointment to one of the SAs?

I would really appreciate any advice.
 
Can you explain to her office how important this is. Maybe they can fax / email the form to her to sign.
Is their another dermatologist in the office that would be willing to sign it?

If these suggestions don't work try calling your medical technician at DoDMERB. Do everything in your power to get the remedial done in a timely manner. A remedial does not always mean a DQ. It's not over, till its over.

The dermatologist might have another doctor "on-call" in her place. Even that using physician might be a way to expedite the completion of your remedial form.

If I am not able to get hold of another dermotoligist is it possible for me to get the form signed by my GP (general practioner)/ physician?

I'm curious bc DS is a junior and started accutane just after the new year. I thought I had read if DS/DS was off if it for 8 was prior to DoDMERB, then it was ok. Were you still on medication when you had your exam, or off of medications than 8 wks, or did you start medication after DoD exam? I'm curious so I'm can be prepared with DS. Good luck & I'm praying you get the response you need and in the timeframe needed. I'm sure you have worked extremely hard to achieve your goal of attending a SA. Prayers!

I'm curious as well. I heard 8 weeks off the medication and you're clear. Do you still need paperwork and signatures from the dermatologists?

Yes, you're right about having to be off the medication at least 8 weeks before you start service or begin your year at a SA.
I was still taking the medication when I took the medical exam, however, I finished my treatment in time. 10 weeks before the SA year begins, close I know, but on time.

If you have listed that you were on the medication while doing the test (which you must as you have to be honest) then all you need is to get a simple form signed by the dermatologist stating that you are now off the medication and that your liver has been functioning fine through out the treatment.
I'm good to go once I send off the form.

Good luck to you and your DS, it's a long process, but worth it, no mattter the outcome.

DD was on accutane even during DoDMERB exam, which of course triggered a remedial. The remedial did take a signed statement by the Doc that she would discontinue treatment by Feb 15, much longer than the required 8 weeks before plebe summer but that was all they needed to see. An MD in the practice with access to the records should be able to sign also. Yes, practice smiling and cajoling the admins at the office, stress how important it is.

I was able to sort it out when I called DODMERB.
My dermatologist won't be available, but I'm able to get the form signed by my GP/Physician who was taking my blood tests.

I finished my treatment on Feb 28. That isn't too late is it?
