PTR Medical Forms

The doctors took a few days filling out my forms, so I just got my shots today...however, the real problem is that I have to come back in 2 days to get the results of the skin test. That is the 15th.

I asked them to make a copy of the forms as they are, with the one shot info filled out, and I figure I'll mail them in ASAP and then mail in/drive (I live in MD, right outside DC) the updated originals when I get them on Friday.

Can anyone offer any advice? Is there a better way of going about doing this?

I just did the same thing, and if you take a certain interpretation of what it says in the PTR packet, we just have to have it in the mail by Friday, which works out quite well. I don't think they'll pull our appointments if it's a day late.
 
I wouldn't worry about it too much. However, I wasn't able to convince my son, who insisted on express mailing the medical forms today because the immunization paper said "due by May 15".
 
I am just waiting on my labs- I am hopefully going to get them tomorrow then I am going to express mail the forms in too. If not I do not think it will be a huge problem if we send they are post marked a day or two after the 15th.
 
Okay, thanks for the support. That makes me feel a LOT better! I was freaking out for a while there. It'll probably be express mail on Fri morn after I get the forms back (thank God I only have one class that day!).
 
ya I'm kind of freaking out too, I have to reschedule my appointment and the earliest is friday morning but I'm not sure yet. Worst case scenario I get in to the doctor early next week. what do you think will happen if my forms are that late ???
 
Another thing. I want to clarify something about The Adult and Pediatric Immunization Questionnaire that goes along with the immunization record. From what I understand, I fill out the front side(questionnaire) and when I go to the doctor for vaccinations/ skin test he fills out the back side. is this correct?
 
I am confused by the dates - in some places it appears that the packet has to be returned by May 15 and in others it appears we have until May 30. It took 4 different visits to our doctor's office to get the immunization form completed and we just got it back yesterday. There is no way it will get to Annapolis by tomorrow....
 
apparantly i had missed this form when looking over all the paperwork several times. the form has two parts one at the top says to bring to i-day yet the bottom mailed in? what happens if it is a few days late?

Send the medical forms by express mail. I was told yesterday (after we sent ours) that it is ok for the forms to arrive later than May 15, but you should make every effort to get them there ASAP.

The rest of the package is due May 30.
 
Did you say formS as in more than one? Other than the immunization form what else is there to send by may 15? Sorry for all the confusion - i dont have the packet with me....
 
11 a (immunization form) and b (the patient registration form). Also c (glasses/contacts prescription info form) if it applies to you.
 
what about the patient questionnaire. does that need to go along with the immunization sheet?
 
I think we fill out the one side of the questionaire and we bring it with us on I-day and the other side is if they have to give us any more vaccinations, etc. But if someone talked to admissions and wants to clarify that would be great!
