The form says that my ALO has to verify that I have no tattoos and he has to sign the form. However, my ALO lives halfway across the country, how can I handle this situation?
 
Not sure why your ALO is so far away, could be you moved, are in college, or a myriad of other reasons, but that is sort of beside the point for now, especially if you will not be able to meet with them before the form deadline.

First, using the USAFA site, find one or two ALOs closest to your current area.
Then, contact your existing ALO and Academy Admissions to see if they would accept his/her affirmation on the form.
If yes, then have admissions and/or your ALO make initial contact with the "new" ALO explaining the situation.
Finally, you contact the new ALO to set up a time/place to meet and complete the form.

Doesn't matter. The two forms/sources are unrelated and used for purposes and by different groups.
One does not replace or supersede the other.

If I recall from a couple of years ago, it is not to verify you have NO tattoos, but which tattoos as well as piercings, you do or don't have.

Even if we have no tattoos we still have to fill it out and scan it in, but it will be pretty much blank. Since USAFA changed the interview process and assigned us to random ALOs, all of our ALOs are very far away.

I emailed the ALO closest to me to set a time so I'd recommend doing that...I assume that's the best way to get it finished?
 
I also had no tattoos and my ALO just signed it electronically. It's not like they make you undress - they take your word for it. I would ask about scanning it to him to sign.
