Scheduling DODMERB question

The instructions to candidates lists 6 items (forms) and says if you complete 3 of them your name is forwarded to DODMERB to schedule the medical exam. One of those items is "names submitted for all the three teacher evaluations."

However, on the portal, the list of items to move forward with DODMERB does not include submitting the teacher names. Would anyone know which is accurate?
 
I think the list on the portal is more accurate. And speaking of DODMERB, please consult with your parents before answering any question with a yes. They will have all the details. You do not want a dq because of an inaccurate childhood memory.

Just wait for the emails.. first email from AFA saying DODMERB will contact you. Then next from DODMERD...log in process .. applications, 81 questions then schedule your appt. I made the calls and help out with the applications. DS had broken arms 2x, 2005 n 2013... those yes has few extra questions. Plus resent X-ray. Eye Dr will try again on Monday. Hoping to see the print out after that!

Thanks! Which 3 items did you complete? How long after USAFA email did you get the one from DODMERB?

I think it was all done but the essays and not all are sent. My DS is at AIM program right now and will also go to Marine leadership n character academy, he’ll be back home in 2 weeks. USAFA n DODMERB email were just a minute apart.
 
DS received two DODMERB emails last week a day apart. I think USNA and USAFA sent at the same time or close enough. He tried to make appointments. Doctor said make eye appointment first. Called eye doctor and they don't have the dodmerb info yet so they said call back.

SF86 is the security questionnaire.
DODMERB is the gateway for medical Q/DQ.

Recommend browsing the DODMERB forum here on SAF, and reading the stickies at the top.

Both places, I am fairly sure. Many applicants to the military report past experimental or one-time drug use. It’s not unusual. The best choice is to be honest. There are dozens of threads on this.

I didn’t take the time to find the most current copy of the DODMERB medical history form, but here’s a link to an older version. The drug and alcohol questions have been around for decades.

But what if I already said no on form 2030?
 
How many days did you guys have to wait for to recieve mail from air force academy for DODMERB, after completing the 3 of the 6 required forms?
  • USAFA Form 146 - Air Force Academy Candidate Personal Data Record
  • USAFA Form 147 - Air Force Academy Candidate Activities Record
  • AF Form 2030 - USAF Drug and Alcohol Abuse Statement
  • USAFA Form 0-501 - Drug and Alcohol Use Circumstances (will only appear if required)
  • USAFA Form O-878 - USAF Academy Candidate Writing Sample
  • USAF Academy Candidate Fitness Assessment
 
FYI- my DS waited about a week after 3 checkmarked items were complete, then he was given notification to start the DodMRB process.

Also, one of his buddies is about the same pace as my DS, and he never got an email from the AFA.

He went to the DoDmets site and established an account and his request was there from the AFA (dated within a day of my DS), and scheduled to go to the exam together. Then they both then established their own DodMERB accounts. It took about 10 days for they process to complete.

Both are now qualified, and my DS’s friend still says he never got an email from the AFA to start the process.

Seems like there might be a glitch somewhere.
