Not true for USN commissioned ships, such as USS GEORGE H.W. BUSH, commissioned in 2009. It’s usually the case, but not always. I’d be interested to know if CG naming protocol differs.

Yes, different for the Coast Guard. I think the info is in the public affairs manual.
