Interview report on check list

Just wondering if there are other applicants here who have completed their interview, but it is not checked off on the application check list. Interview was completed on November 5th and DD recently emailed her AO to make sure the interview report had been received and in her file, but never received a response from him.
Anyone else have this happen?

DS interview was not checked off and got email confirmation of appointment without interview being checked off.
