    Just got my hands on DoDMERB DD Form 2492. For Tree Nut history, do Q's #13 (Have any allergies) and #60 (Adverse reaction to...foods) both have to be checked yes? It seems repetitive. Explanation on page 2 would be the same for both questions, so just not sure if both should be checked or...
