If you can get your own doctor is better because you can see the results first before sending them in. If you are not able to do this monetary wise then do it with them but it could take about a two weeks for them to find a doctor for you.

What was the issue with the knee?? I just got a waiver for a past issue on my knees and I never got surgery but mine was 6 years ago though.

It means just that, that they received the paperwork on their offices and are looking at it. You should either expect a Qualified a DQ or a remedial.

Be sure that your therapist does not write that you have depression or any mental illness in his OWN notes because you can be stating that you don't have those issues but he/she might think differently. Also if you get prescribed it could be an issue, I don't have much info on this but just...
