I'm a bit confused on this form, is it a background check or do we just need to get the two questions answered with a signature? I can get a criminal background check by paying for it but I do not know if it will be accepted since it is not the form WP gave.

I uploaded my birth certificate as soon as I accepted my appointment a little over 3 weeks ago and it has not been received yet. Is it normal for it to take that long? If I uploaded it incorrectly, would I be notified somehow? Can I upload it again? If anyone can help, I'd greatly appreciate it!
