I recently got a remedial R251.12 Athletics/ Recreational participation questionnaire and R252.90 Othropedic Questionnaire. I found the forms for the questionnaire and was wondering if I have to mail these forms or send them through email.

A few weeks ago, I got a remedial because my provider forgot to fill out a section. My providers sent in the missing information to DoDmerb and I was wondering how long it takes for my medical to be completed.

So my status now is remedial requested and got the code R251.00 because the doctor forgot to notate something. On the letter it says required administrative AMI. Would I have to contact concorde for this?

A week ago it said that my case status was at complete hold and then I checked today and it says that my case status is incomplete closed. I went to the phy and opto exams and both exams were submittted to dodmerb. Anybody else have this issue?

I received a scholarship for NASS. For me they took up to four days to respond. Once you fill out the form for the scholarship application you don't have to worry about the payment due date. They should respond to you soon
