    I also would recommend having a civilian school back-up where you could go without being required, by finances, or anything else, to do ROTC, just in case something medical turns up. This could be a separate school, or the same one, however it works out.

    You won't get it in the mail. Did you receive the 'Instructions to Appointees' packet? If you read everything in it, some piece of paper (don't have it handy) gives you a link to the form. If you didn't get that packet, I would probably call admissions and ask about it.
